The ASL of Foggia has implemented an integrated care program for taking charge and management of chronic patients in the area in order to ensure adequate levels of health services in the Province of Foggia, especially for elderly, frail and chronic patients.
The objectives of the initiative can be summarized in the following points:
1) offer a rational care path that adheres to the guidelines
national and local;
2) promote adherence to follow-up by the chronic patient by making i
assistance services more easily usable in the territory of residence;
3) avoid the mobility of chronic patients and the use of hospitalization
inappropriate;
4) reduce the wait for the patients involved, also affecting the "waiting lists"
business;
5) offer an efficient level of assistance to the entire population.
