The Zelo Sade Platform is a Mobile Platform to support decision-making in the management of the care of the dependent elderly person with two applications, one for the Health Professional and another for the Caregiver/Family member, allowing interaction between the users of the two applications, facilitating communication between them and the integration of information on the daily care of the elderly.
The information recorded by health professionals reflects on the care of the dependent elderly person, as the necessary information for daily care by caregivers/family members is generated.
In this way, the applications (Health Professional and Caregiver/Family Member) work in an integrated manner, generating relevant data in the context of the health of the elderly.
The main features of the Zelo (Caregiver/Family member) application are:
- Personal data;
- Visualization of clinical and functional assessment;
- Monitoring the Care Routine;
- Monitoring of registered medicines;
- Visualization of complications;
- Reminders and reports about care for the elderly;
- Interaction and communication with the care team (health professionals, caregivers and family members);
- Informational videos;
- Report.
Thus, the use of the two applications in the context of the health of the dependent elderly person allows:
To Health Professionals:
- Have access to the elderly person's information, including notifications and alerts;
- Carry out Clinical and Functional Assessment of the elderly;
- Propose a Care Plan for the elderly, guiding the actions and instructions necessary for the Care Routine performed by caregivers;
- Register medicines;
- Inform or be informed of suspected abuse;
- Record home visits;
- Have access to health professionals involved in the care of the elderly;
- Prevent errors/damage in the care of the elderly through the interaction between health professionals, caregivers and family members;
- To be informed, as soon as possible, of the complications of the elderly person, in view of the interaction between the professionals of the health team.
To Caregivers/Family Members:
- Have access to the information necessary for the care of the dependent elderly person;
- Have help through reminders so as not to forget the necessary actions for the daily care of the dependent elderly person, such as care instructions and medications;
- Have support from health professionals when necessary; -Have access to Clinical and Functional Assessment performed by health professionals;
- Have access to the Care Plan prepared by health professionals through the care proposed in the Care Routine;
- Monitor the actions of health professionals involved in the care of the elderly.
